The Forkhead Transcription Factor Foxi1 Is a Master Regulator of Vacuolar H+-ATPase Proton Pump Subunits in the Inner Ear, Kidney and Epididymis

Abstract: The vacuolar H+-ATPase dependent transport of protons across cytoplasmic membranes in FORE (forkhead related) cells of endolymphatic epithelium in the inner ear, intercalated cells of collecting ducts in the kidney and in narrow and clear cells of epididymis require expression of several subunits that assemble into a functional multimeric proton pump. We demonstrate that expression of four such subunits A1, B1, E2 and a4 all co-localize with the forkhead transcription factor Foxi1 in a subset of epithelial cells at these three locations. In cells, of such epithelia, that lack Foxi1 we fail to identify any expression of A1, B1, E2 and a4 demonstrating an important role for the transcription factor Foxi1 in regulating subunit availability. Promoter reporter experiments, electrophoretic mobility shift assays (EMSA) and site directed mutagenesis demonstrate that a Foxi1 expression vector can trans-activate an a4-promoter reporter construct in a dose dependent manner. Furthermore, we demonstrate using chromatin immunoprecipitation (ChIP) assays that Foxi1-dependent activation to a large extent depends on cis-elements at position ?561/?547 in the a4 promoter. Thus, we provide evidence that Foxi1 is necessary for expression of at least four subunits in three different epithelia and most likely is a major determinant for proper assembly of a functional vacuolar H+-ATPase complex at these locations.
Selective Electrocatalytic Hydrogenation of Linolenic Acid on and Catalysts

Abstract: Electrochemical hydrogenation of linolenic acid as a model for polyunsaturated acids was studied on Pd and Pd/Al2O3 catalysts in acidic and alkaline media. The results are presented in terms of number of double bonds in the polyunsaturated fatty acid and interpreted in terms of the adsorption capacity of the catalysts in these media. The highest hydrogenation yield was obtained with Pd/Al2O3 at pH 13, in good correlation with the adsorption power of linolenic acid and its first hydrogenation product, linoleic acid, measured in this solution. A preliminary electrochemical hydrogenation study was conducted on Pd/Al2O3 catalyst containing Co, in the optimum electrolysis conditions, showing a cooperative effect of the noble metals regarding the cis/trans selectivity with preferential formation of cis-oriented monounsaturated compound. All the products were characterized by gas chromatography after derivatization of the samples; fifteen cis-trans isomers of monounsaturated fatty acid which could be identified are presented here.
Assessing the evolution of primary healthcare organizations and their performance (2005-2010) in two regions of Québec province: Montréal and Montérégie

Abstract: In early 2005 we conducted a study in the two most populous regions of Quebec province (Montreal and Montérégie) which assessed the association between prevailing models of primary healthcare (PHC) and population-level experience of care. The goal of the present research project is to track the evolution of PHC organizational models and their relative performance through the reform process (from 2005 until 2010) and to assess factors at the organizational and contextual levels that are associated with the transformation of PHC organizations and their performance.This study will consist of three interrelated surveys, hierarchically nested. The first survey is a population-based survey of randomly-selected adults from two populous regions in the province of Quebec. This survey will assess the current affiliation of people with PHC organizations, their level of utilization of healthcare services, attributes of their experience of care, reception of preventive and curative services and perception of unmet needs for care. The second survey is an organizational survey of PHC organizations assessing aspects related to their vision, organizational structure, level of resources, and clinical practice characteristics. This information will serve to develop a taxonomy of organizations using a mixed methods approach of factorial analysis and principal component analysis. The third survey is an assessment of the organizational context in which PHC organizations are evolving. The five year prospective period will serve as a natural experiment to assess contextual and organizational factors (in 2005) associated with migration of PHC organizational models into new forms or models (in 2010) and assess the impact of this evolution on the performance of PHC.The results of this study will shed light on changes brought about in the organization of PHC and on factors associated with these changes.In early 2005 we conducted a study in the two most populous regions of Québec province (Mont

Abstract: The ultimate active constituents of the living medium, membranes, ions and molecules, are at the level of the nanometer. Their interactions thus require a quantum processing. The characteristic Action A, linked to the "quantum objects" : ions, radicals, water molecule... of the living medium, has an average value of A ~= 14.10-34 J.s or A ~= 2 h . It is thus strictly impossible to formulate a realistic "classical " theory of membrane electrodynamics. The transfer of the sodium ion - among others - could be then ensured under the action of a Tunnel effect, (with Hartman?s mechanism) specific to the channel and the ion transferred.
From physical mobility to psychological mobilization: The benefits of changing location and caregiver in a mobile team for adolescent psychiatry  [PDF]

We have developed a mobile team for adolescents with psychological difficulties that can intervene within 48 hours, if necessary. Practically, caregivers working in pairs go wherever the adolescents are (home, General Practioner’s office, etc.), travelling in a motorhome designed as a mobile office. This method allows us to approach more closely the adolescents and their families in the here and now that corresponds to our societal evolution. Most of the adolescents seen by the mobile team do not express any explicit demand but the demand is expressed by the professionals in direct contact with these adolescents in difficulty (school professionals, General Practitioners, etc.), who alert the mobile team and solicit the adolescents’ parents to call the mobile team. One characteristic of this mobile team is the mobility of setting (by changing locations, including the use of a mobile office, and caregivers, so that multiple representations are mobilized). Statistical analyses conducted on 520 adolescents followed by the mobile team showed that the first meetings tend to take place in the family’s home, subsequently transferring to the mobile office and finally in the community mental health centres. There is therefore a transition from the living space (a place that is part of the here and now) to the caregiving space (a space of thought and for thinking), with the mobile office serving as an intermediate step in this movement towards accessing care. Furthermore, there was a significant association between changes of locations and continuity of care. Thus, there were significantly more breakdowns in continuity when the consultations all take place in a single location (family home, mobile office, community centre). The results and their therapeutic implications will be discussed in this article, especially with regard to the key role played by physical movement in psychological mobilization, and by plurality of representations associated with different locations and caregivers.

Work and health of Ile-de-France residents: survey results, health care and prevention stakes

Abstract: Aim: Describing the health state of the population living in the Region le-de-France according to occupational exposures and how to consider these elements in clinical practice and organisation of care.Methods: A Health Survey has been conducted during the decade 2002-2003 among the general population, with the aim to appreciate health care consumption and morbidity. At the national level 16,800 households (40,000 people) replied to this survey, among them 3,800 households (9,200 people) in Ile-de-France, including 10,708 people working in the provinces and 3,483 in Ile-de-France.Results: The working population is healthier than unemployed people. On the other hand, mental health is less satisfying, especially in Ile-de-France, where social and health inequalities are more obvious among women. These health problems are according to occupationalexposure to organizational and time constraints and to physical and chemical damages. This trend is reinforced by perception of hard and painful working conditions. These long-lasting effects with long-termsick-leaves and work incapacities are more frequent in craftsmen and workpeople. Conclusion: It is necessary to : – Raise the awareness of companies and other actors concerning occupational and psycho-social risks prevention,– Optimise at all ages an appropriate adequacy between professional activities and people’s capacity to face them.– Adapt working situations in order to favour keeping people in their job and to take into account this work dimension in patients’ medical course evolution.Prat Organ Soins 2009;(2):113-24

Abstract: Carbonic anhydrases are zinc metalloenzymes that catalyze the reversible hydration of CO(2) to form HCO(3)(-) and protons according to the following reaction: CO(2) + H(2)O = H(2)CO(3) = HCO(3)(-) + H(+). The first reaction is catalyzed by carbonic anhydrase and the second reaction occurs instantaneously. The carbonic anhydrase (CA) gene family includes ten enzymatically active members, which are major players in many physiological processes, including renal and male reproductive tract acidification, bone resorption, respiration, gluconeogenesis, signal transduction, and formation of gastric acid. The newly identified CA IX (previously called MN) and CA XII are related to cell proliferation and oncogenesis. Carbonic anhydrase isozymes have different kinetic properties and they are present in various tissues and in various cell compartments. CA I, II, III and VII are cytoplasmic, CA V is mitochondrial, and CA VI is present in salivary secretions. CA IV, IX, XII and XIV are membrane proteins: CA IV is a glycosyl-phosphatidylinositol-anchored protein, and CA IX, XII and XIV are transmembrane proteins. The present work will focus on the roles of CA II and CA IV in transepithelial proton secretion and bicarbonate reabsorption processes. The localization of these isoforms in selected epithelia that are involved in net acid/base transport, such as kidney proximal tubules and collecting ducts, and tubules from the male reproductive tract will be reviewed.
