Adjuvant Use of Ivabradine in Acute Heart Failure due to Myocarditis

Abstract: We report two cases of young men in whom acute heart failure due to myocarditis was diagnosed. The patients had been transferred to the intensive care unit (ICU) with commencing symptoms of acute heart failure and consecutive multiorgan failure for further treatment and to evaluate the indication for implantation of a ventricular assist device or for high urgent orthotopic heart transplantation. In both patients, the -channel inhibitor ivabradine was administered off-label to provide selective heart rate reduction, and thus support hemodynamic stabilization. Though currently considered off-label use in patients suffering from severe hypotension and acute heart failure, the use of ivabradine may beneficially influence outcome by allowing optimization of the patient's heart rate concomitant to initial measures of clinical stabilization.

Abstract: The Versajet? Hydrosurgery System utilizes a reusable power console with foot pedal activation, disposable handpiece and tubing assembly in conjunction with sterile saline and standard waste receptacle. The purpose of this paper is to report our experiences with this instrument in debridement of a variety of wounds prior to final reconstructive surgery. Technical details and pitfalls are discussed to facilitate clinical use.Efficient, safe and fast debridement was achieved in all patients using the hydrosurgery system. The actual time the hydrosurgery system was used for debridement averaged as 15.5 minutes. In ten patients, an adequately debrided wound bed was achieved with a single operative procedure, in four patients; two stages were required prior to reconstructive surgery. In one patient with recurrent sacral-iscial pressure sore, two debridements were carried out followed by long term vacuum assisted closure. The postoperative course was uneventful in all patients, but in three with a minor breakdown of the skin graft, which eventually healed with no surgical intervention.As a result of our clinical experience, the Versajet? enables surgeon to precisely target damaged and necrotic tissue and spare viable tissue. This modality may be a useful alternative tool for soft tissue debridement in certain cases. However, further studies are required to investigate its cost-effectiveness in wound management.Surgical debridement of necrotic tissue is an essential part of wound care prior to any reconstructive options. Sharp techniques utilized for this purpose have been the mainstay and are commonly used in combination with pulsed lavage and/or irrigation.Recently, a new device, the Versajet? Hydrosurgery system, has been described for debridement. The Versajet? hydrosurgery system utilizes high fluid technology for wound debridement. Abstract: In his De vulgari eloquentia (1306) Dante deals with the twofold problem of the origin of language and of the primary structure of the language of Adam, which Dante, such as everyone in the medieval period, identified as Hebrew. The Hebrew structural features give the normative paradigm for the elaboration of a taxonomy of the historical languages and of the Italian dialects, subdivided on the ground of their nobility . In such context, he also theorizes about the connection between the development of the job division and the linguistic differentiation in the mankind, starting from an unheard-of reading of the biblical episode of the tower of Babel. Dante s thought is particularly concerned with the difference between forma locutionis (comparable to Saussure s langue) and actus locutionis (comparable to Saussure s parole). Comparing Hebrew, Latin and the Illustrious Vulgar Language , Dante outlines a doctrine where the poetic language is interpreted as an attempt to take back the historical idioms to the nobility which was prerogative of the Edenic language.

Abstract: This report explores the use of machine learning techniques to accurately predict travel times in city streets and highways using floating car data (location information of user vehicles on a road network). The aim of this report is twofold, first we present a general architecture of solving this problem, then present and evaluate few techniques on real floating car data gathered over a month on a 5 Km highway in New Delhi.

Abstract: Advanced control of distributed energy resources at the consumer level requires full situational awareness of the the distribution system. One important problem is that of feeder topology identification, due to changing switch configurations, given a sparse number of measurements. We formulate the problem for residential feeders as a spanning tree identification problem over a general graph. Given a set of power flow measurements and load pseudo measurements, we show that the underlying graph structure is crucial in defining identifiability of the correct spanning tree on the graph. First we solve the deterministic case of known true loads and measured power flow. We show that the placement of sensors on the network alone determine whether the set of spanning trees can be correctly identified. In the stochastic case where loads are in the form of noisy forecasts, we present a locally optimal sensor placement algorithm.
Short Term Electricity Load Forecasting on Varying Levels of Aggregation

Abstract: We propose a simple empirical scaling law that describes load forecasting accuracy at different levels of aggregation. The model is justified based on a simple decomposition of individual consumption patterns. We show that for different forecasting methods and horizons, aggregating more customers improves the relative forecasting performance up to specific point. Beyond this point, no more improvement in relative performance can be obtained.

Abstract: Cln3Δex7/8 mice harbor the most common genetic defect causing juvenile neuronal ceroid lipofuscinosis (JNCL), an autosomal recessive disease involving seizures, visual, motor and cognitive decline, and premature death. Here, to more thoroughly investigate the manifestations of the common JNCL mutation, we performed a broad phenotyping study of Cln3Δex7/8 mice. Homozygous Cln3Δex7/8 mice, congenic on a C57BL/6N background, displayed subtle deficits in sensory and motor tasks at 10–14 weeks of age. Homozygous Cln3Δex7/8 mice also displayed electroretinographic changes reflecting cone function deficits past 5 months of age and a progressive decline of retinal post-receptoral function. Metabolic analysis revealed increases in rectal body temperature and minimum oxygen consumption in 12–13 week old homozygous Cln3Δex7/8mice, which were also seen to a lesser extent in heterozygous Cln3Δex7/8 mice. Heart weight was slightly increased at 20 weeks of age, but no significant differences were observed in cardiac function in young adults. In a comprehensive blood analysis at 15–16 weeks of age, serum ferritin concentrations, mean corpuscular volume of red blood cells (MCV), and reticulocyte counts were reproducibly increased in homozygous Cln3Δex7/8 mice, and male homozygotes had a relative T-cell deficiency, suggesting alterations in hematopoiesis. Finally, consistent with findings in JNCL patients, vacuolated peripheral blood lymphocytes were observed in homozygous Cln3Δex7/8 neonates, and to a greater extent in older animals. Early onset, severe vacuolation in clear cells of the epididymis of male homozygous Cln3Δex7/8 mice was also observed. These data highlight additional organ systems in which to study CLN3 function, and early phenotypes have been established in homozygous Cln3Δex7/8 mice that merit further study for JNCL biomarker development.
A Functional Architecture of Optic Flow in the Inferior Parietal Lobule of the Behaving Monkey

Abstract: The representation of navigational optic flow across the inferior parietal lobule was assessed using optical imaging of intrinsic signals in behaving monkeys. The exposed cortex, corresponding to the dorsal-most portion of areas 7a and dorsal prelunate (DP), was imaged in two hemispheres of two rhesus monkeys. The monkeys actively attended to changes in motion stimuli while fixating. Radial expansion and contraction, and rotation clockwise and counter-clockwise optic flow stimuli were presented concentric to the fixation point at two angles of gaze to assess the interrelationship between the eye position and optic flow signal. The cortical response depended upon the type of flow and was modulated by eye position. The optic flow selectivity was embedded in a patchy architecture within the gain field architecture. All four optic flow stimuli tested were represented in areas 7a and DP. The location of the patches varied across days. However the spatial periodicity of the patches remained constant across days at ~950 and 1100 μm for the two animals examined. These optical recordings agree with previous electrophysiological studies of area 7a, and provide new evidence for flow selectivity in DP and a fine scale description of its cortical topography. That the functional architectures for optic flow can change over time was unexpected. These and earlier results also from inferior parietal lobule support the inclusion of both static and dynamic functional architectures that define association cortical areas and ultimately support complex cognitive function.

Abstract: Two integrases inhibitors, raltegravir and elvitegravir, have now been approved by regulatory agencies for use in the treatment of HIV-infected patients; and the approval of a third such drug, dolutegravir, is expected during 2013 on the basis of several phase 3 clinical trials. The advent of this new class of antiretroviral (ARV) medications represents a major advance in the management of HIV infection, and each of these three drugs can be expected to continue to be an important component of ARV combination regimens.
